45-002 27.6 11/12/1999   YES NO NO NO Former vehicle decontamination area located on mesa edge near county skateboard park.   No evidence of former site remains. Area mostly covered with asphal, with some areas of grass and ponderosa pines. Decontamination facility located on mesa's edge. YES NO Area has been recontoured and PRS now covered with asphalt and grass. Sheet flow runoff over mesa edge into tributary channel.     Sheet flow runoff from site terminates into tributary channel and into Acid Canyon. None observed.  Area stable with asphalt and mature canopy cover. NO No nearby structural impacts. NO No operational impact. NO No run-on from upslope sources. Veenis, Steve     NO Site originally evaluated on 10/07/1997, and was submitted to NMED on 1/7/98.  Updated information required a re-evaluation of the site.
45-003 27.6 11/12/1999   YES NO NO NO Former industrial waste sewer lines associated with the TA-45 treatment facility.  Currently used as skateboard park. PRS mostly under asphalt with some areas of grasses. Waste line ran parallel with existing fence line.  Area mostly flat with gentle slope towards the north. YES NO Area has been recontoured and PRS now covered with asphalt and grass.  Sheet flow runoff over mesa edge into tributary channel.     Sheet flow runof from site terminates into tributary channel and into Acid Canyon. None observed.  Area stable with asphalt and mature canopy cover. NO No nearby structural impacts. NO No operational impact. NO No run-on from upslope sources. Veenis, Steve     NO Site originally evaluated on 9/16/1997, and was submitted to NMED on 1/7/98.  Updated information required a re-evaluation of the site.
45-004 50.2 11/12/1999 45-3 YES YES NO NO PRS is a drain line off of an old pump station, north of the county skateboard park.  The PRS boundary includes the area north of the pump station as well as a small natural drainage that discharges into Acid Canyon.  The drain pipe has been removed. Both ground and canopy exist, but do not provide total coverage of the area soils. Less than 10% between the pump station and natural drainage.  10% to 30% within the natural drainage. YES NO Sheet flow runoff from the site directly into drainage channel.     Runoff terminates into tributary channel and into Acid Canyon. No erosional features, but sheet flow erosion can be inferred from the evidence of flow at site. NO Skateboard park parking lot slopes away from site. NO No operational impact. YES A portion of the PRS boundary lies within a natural drainage area. Veenis, Steve Slope stabilization adjacent to the PRS should be evaluated.   NO Site originally assessed on 9/16/1997, and was submitted to the NMED on 1/7/98.  Updated information required a re-evaluation of the site.  Erosion is occurring adjacent (west of) the PRS due to construction of the skateboard park.
